The number of people with long-term conditions and multi-morbidities is rising faster than capacity to manage them. Better self-management and remote care with monitoring capabilities can free up capacity and support the creation of sustainable, quality services that can be delivered remotely and in a manner better suited to service users and maximise the capacity and impact of scarce \u201cface to face\u201d clinical time. <\/p>\n\n\n\n

As part of an ongoing programme of work this project aims to build a patient facing digital platform across the Greater Manchester single-provider regional neurology service (including MS, Headache and Epilepsy) and integrate this with existing electronic records.   <\/p>\n\n\n\n

The UoS will work with patients and Health Care Professionals of the Northern Care Alliance to: <\/p>\n\n\n\n

    \n
  1. Assess readiness for change by examining the barriers and facilitators to implementing digital technology at this scale.\u00a0<\/li>\n<\/ol>\n\n\n\n
      \n
    1. Evaluate the facilitators and barriers as the technology is deployed.\u00a0\u00a0<\/li>\n<\/ol>\n\n\n\n
        \n
      1. Analyse patient engagement with the system pilot testing PREMs and PROMs and tools to measure self-management.\u00a0<\/li>\n<\/ol>\n\n\n\n
          \n
        1. Examine the usefulness and acceptability of the system and required processes for clinical staff.\u00a0\u00a0<\/li>\n<\/ol>\n\n\n\n
