Information:

The cognitive failures questionnaire is a 25 item questionnaire regarding everyday mistakes people make.

Items are scored on a 4-0 scale, with the total scale score calculated as a sum of the items.

There are sub-scores that can be calculated for: forgetfulness, distractibility and false triggering.
